Are you a carer?

A carer is anyone who provides care or support for another person. It may be full time or just providing more limited support in the form of doing the weekly shopping.

If you are a carer please let us know by completing a carer registration form at reception. This will allow clinicians to know that as a carer you are under a greater pressure than would normally be expected.

Please note that if you are a carer by occupation E.g. work in a care home you do not need to complete a carer registration form.

If you provide a substantial amount of support for the person that you are caring for, as a carer you are entitled by virtue of the Carers & Disabled Children's Act 2000, to a Carers Assessment of your own needs.

To request a Carers Assessment, where ever you live in Northamptonshire, contact Northamptonshire County Council, Community services "Adult Care Team" on 0845 124 4500.
